The form can also be obtained by contacting Equiniti on 0371 384 2250* (+44 121 415 7047 if calling from outside the United Kingdom) and via the shareholder website at www.shareview.co.uk. Alternatively, you will find one attached to the tax voucher of your last dividend payment.
